Provider surveys compile data from physicians, hospitals and other health care providers. These surveys typically obtain information from case records selected:
  • As a sample of all of a set of medical transactions
  • As a sample of patients or
  • As patients with a particular condition--for example
    • those who have used illicit substances,
    • those with a particular disease [AIDS, cancer,] or
    • those who have received a particular procedure
