The American Physical Therapy Association produces the Outpatient Physical Therapy Improvement in Movement Assessement Log (OPTIMAL).

Purpose
OPTIMAL enables clinicians to document patients' automatic exception to the outpatient therapy cap for payment. The Centers for Medicare and Medicaid Services recommended OPTIMAL as a tool for documenting objective and measurable physical function of patients.

Description
OPTIMAL measures patients' difficulty and self-confidence in performing 21 specific functional movements.
