On behalf of the International Health Terminology Standards Development Organisation (IHTSDO), NLM is pleased to announce the release of the Candidate Baseline of General Practitioner/Family Practitioner (GP/FP) Reference Sets and SNOMED CT to International Classification of Primary Care (ICPC-2) Map. This is an update to the previously released baseline work, resulting from the harmonization agreement between the IHTSDO and the World Organization of Family Doctors (WONCA).

The purpose of the GP/FP subset is to provide frequently used SNOMED CT concepts for the reason for encounter and health issue data fields within general/family practice electronic health records. The purpose of the SNOMED CT GP/FP subset to ICPC-2 map is to allow for granular concepts to be recorded by GPs/FPs at the point of care using SNOMED CT, and the subsequent analysis and reporting utilizing the internationally recognized ICPC-2 classification.

The release is aligned to, and should be used with, the July 2015 International SNOMED CT Release. The GP/FP subset to ICPC-2 map is still a baseline release but it is planned to be included with future International SNOMED CT releases, starting in January 2016.
