This depends on expectations about which reporters would be most accurate.

For example, demographic characteristics—such as race, ethnicity or income might be more accurately reported by patients—but information about diseases might be more accurate if compiled directly from hospital or physician records.

How frequently will the information be collected? This depends on assumptions regarding the rate at which conditions change.
