Use this form to request additions or changes to bibliographic data in LocatorPlus. Titles will be added if they are substantive journals which are currently owned, licensed (if electronic), and offered through interlibrary loan by biomedical institutions to support biomedicine, health care delivery, research, education, and management. Titles reported for addition to or modification of bibliographic data in NLM's LocatorPlus for use in DOCLINE must be accompanied by supporting bibliographic documentation such as OCLC or LC records (if available), or a photocopy of the title page and cover of the journal in question if no bibliographic record is available.

NLM uses a single record cataloging approach for journals issued in multiple formats. Only one OCLC number is recorded in each bibliographic record. Do not request the addition of OCLC numbers for different formats. Please use one form for each title and limit your requests to no more than 10 titles/month.

When requesting new titles be added, please be sure you have searched DOCLINE and/or LocatorPlus Catalog using multiple search options including Title, ISSN, or title abbreviation before submitting this form. ****SIEBEL FORM****
